Registration
Student enrollment packets are available in the front office. To make the process go quickly, please provide proof of residence, proof of age, and immunization records for each student enrolling:
- Proof of Residence Examples
- Parent Photo ID
- Rental Agreement
- Utility Bill
- County/CIty Paperwork
- Medical Documents
- Proof of Age Examples
- Birth Certificate
- Immunization Records
- Polio (OPV or IPV) – 4 doses
- 3 doses ok if one was given on or after 4th birthday
- Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or TD) – 5 Doses
- 4 doses are acceptable if one was given on or after the 4th birthday
- 3 doses are acceptable if one was given on or after the 7th birthday
- Measles, Mumps, and Rubella (MMR) – 2 doses
- Hepatitis B – 3 Doses
- Varicella – 2 Doses
- Any legal court documents showing custody or restraining orders you may have.

*All paperwork must be filled out and returned to the school office at one time before a student can start
